    Staff, Faculty and Student Reporting

      • Matters that meet specific jurisdictional elements of Title IX are governed by these policies and procedures.
      • CARE Team: The Campus Assessment, Response, and Evaluation Team (CARE Team) engages in various approaches to identify and assess students who are distressed or may exhibit concerning behaviors. Faculty, staff, and students should use this form to notify the CARE Team about any student concerns. This could be a wide variety of things: from a student who stopped going to class to a student experiencing suicide ideation.

      We encourage reporting of any incident, no matter the basis or the parties involved. Below are the resources available for reporting an incident.

Faculty, staff, and students should use this form to notify the CARE Team about any student concerns. This could be a wide variety of things: from a student who stopped going to class to a student experiencing suicide ideation.
        • The Dean of Students office is responsible for upholding the Code of Student Conduct as well as assisting students if they have a complaint against a faculty member within the context of the classroom (grade appeals, other general academic issues). Students can call or email with questions and to receive assistance.

        We encourage reporting of any incident, no matter the basis or the parties involved. Below are the resources available for reporting an incident.
