Form Instructions
The Chemical Waste Pickup document is a PDF form. Review the following instructions to ensure the form is completed and submitted correctly:
- Save the PDF to your computer.
- On the first page, enter information in each field at the top. The second page will automatically fill.
- Select school (PU or IU).
- There is space on the 2-page form for 8 types of waste. If you have more than 8, submit an additional form.
- Each box has a space for the number of containers, container size, amount in container, and contents.
- To submit, click the green “Send” button on the top of the first page of the form.
- The form will open your campus email and attach the PDF automatically.
- You will need to send the email in order to submit.
- Alternatively, you may attach the PDF in an email to [email protected].
