Students are responsible for providing an up-to-date and accurate record of personal information to the University. The Office of the Registrar is responsible for maintaining this information for the University.

You may change your address and phone number using goPFW under Personal Information. You can also change your address and phone number by completing a Personal Information Change Form in person at the registrar’s office. All students are required to have an active permanent address on file.

If your name has been legally changed or is inaccurate in our system, you may complete the Change of Name Form to request a change. Your legal name according to the federal government and the state of Indiana is what appears on your academic record and transcripts.
You will need to include a scan or photo of legal documentation of your name. A link will be provided at the end of the survey form for you to securely upload your documentation.
Acceptable forms of documentation include:
- Driver's license or state issued photo ID
- Social Security card
- Permanent Resident card (Green card)
- Passport
- Divorce decree
- Court order
Former IPFW students needing their IU record updated should complete a request through IU Student Central.

You can request a preferred name by completing the Registrar’s Preferred Name Change Form . You do not need to provide documentation. The university has the right to revoke or deny a preferred name request if it contains inappropriate or offensive language, avoidance of legal obligation, or is being used for misrepresentation. Please allow 24–48 hours for the request to be processed. You may change your preferred name only once per semester. You may update only your first name with a preferred name. You may not update your last name.
The university will continue to make necessary changes as the implementation of the preferred name process continues and where this information is made available. While your preferred name will be used whenever possible, there are places and documents where the university will continue to use your legal name for business and legal purposes. These include the following:
- Admissions
- Housing contracts
- Student accounts and university housing bills
- Financial aid
- Enrollment verification
- Transcript
- Teacher licensure records
- Government employment forms
- Governmental tax forms 1098-T and W-2
- Paychecks
- University Police
If you are utilizing your preferred name, you should always be prepared to reference your legal name as well as provide your university identification when necessary. To change your legal name, complete a Change of Name Form.

You will be assigned a nine-digit number, typically beginning with either 900 or 999, as your student identification number. It will be used to identify records within Purdue Fort Wayne and has no significance outside Purdue Fort Wayne. It will not be provided to external agencies or individuals except in accordance with university policy on release of student information.

Purdue Fort Wayne does not use your Social Security number as your student identification number except for those purposes required by law or governmental agencies (i.e., Internal Revenue Service and Free Application for Federal Student Aid).

You must have legal documentation (i.e., driver's license or state issued photo ID, Social Security card, Permanent Resident card (Green card), passport, divorce decree, court order) to make a correction to the spelling of your name. File a Change of Name Form [PDF] and select Correction as the "Reason for Change". Submit the form and a copy of legal documentation at the Enrollment Services desk in the main lobby of Kettler Hall or send your request and a copy of legal documentation by mail to:

Directory information is student information which may be disclosed or publicly shared. The directory at Purdue Fort Wayne includes this information:
- Name
- Address
- Phone number
- Email address
- Class standing
- School or division
- Major
- Dates attended
- Current enrollment status
- Degrees and awards
- Recognized activities or sports
- Information related to athletic-team participation

Students who are currently registered may declare their directory information confidential by completing a Request to Prevent Disclosure of Directory Information Form available on the Information for Students and Parents FERPA page or at the registrar’s office.

Bills are sent to your Purdue Fort Wayne email address. You will not receive a paper bill at your mailing address. If you prefer to receive your bill at another email address, go to goPFW and set up email forwarding to your preferred email address.
Grades are sent to Purdue Fort Wayne students from the main campuses in Bloomington or West Lafayette. If you are not receiving your grades, verify that your mailing address is correct in goPFW or in person at the registrar’s office. If your mailing address is correct in the Purdue Fort Wayne system, request that the registrar’s office verify that your mailing address is correct in the Indiana University system or the Purdue University system.

MY RECORDS HAVE BEEN SUBPOENAED, BUT I DON’T WANT ANYONE TO HAVE THEM. HOW CAN I PREVENT THE UNIVERSITY FROM TURNING OVER MY RECORDS TO THE COURT?
MY RECORDS HAVE BEEN SUBPOENAED, BUT I DON’T WANT ANYONE TO HAVE THEM. HOW CAN I PREVENT THE UNIVERSITY FROM TURNING OVER MY RECORDS TO THE COURT?
The university must comply with any and all subpoenas for records, or it may be found to be in contempt of court. A subpoena for records can be quashed, but the quash is a motion of the court. Consult with your attorney. If you notify us that proceedings for a quash are in progress, you must subsequently forward to us the court motion to quash. If the university does not receive official court documents of a quash, it is bound to comply with the demands of the original subpoena.
