If you are a Collegiate Connection student who took classes before fall 2018, you are are an Indiana University–Purdue University Fort Wayne student. If you are a new student as of fall 2018, you are a Purdue University Fort Wayne student.
If you’re currently taking a dual-credit course, the credit and grade will not be recorded on your official transcript until after you complete the class. There’s a box on the form to state if you want the official transcript held until dual-credit courses have been recorded.
There is an $8 fee for a paper copy of a transcript; the charge of electronic PDF copy is $13. You may specify either option on the request form.
You can request your transcript in person at the Information Desk, located in the first-floor lobby of Kettler Hall. Be sure to bring a state-issued photo ID. Your parent or guardian may not request your transcript.
Postal Mail or Faxed Requests
You may also mail or fax your transcript requests. Please allow up to 10 business days for processing. The forms must be completed, printed, signed, and mailed or faxed to the Office of the Registrar. Due to security and privacy concerns, Purdue Fort Wayne will not fax outgoing transcripts.
