April 14, 2021 1:30 PM
From: Inside Purdue Fort Wayne
To: Students, Faculty, and Staff
Purdue University Fort Wayne is pleased to announce that it has teamed with Walmart Pharmacy to provide free COVID-19 vaccinations to students, faculty, and staff. This opportunity is also open to employees and students of Indiana University Fort Wayne. Appointments for first shots of the Moderna vaccine will be available on the following days:
- Tuesday, April 20, from 9 a.m. to 4 p.m. in the Walb Classic Ballroom
- Wednesday, April 21, from 10 a.m. to 6 p.m. in the Auer Hall lobby of the Rhinehart Music Center
In the coming days, you will receive a text message and/or an email from the Indiana Department of Health ([email protected]) with a unique link to schedule an appointment at the campus vaccination site. The link in this text/email is unique to you and should not be shared.
After scheduling, you will have the opportunity to complete registration online before your appointment, including submitting your insurance information. We strongly encourage you to do this to speed up the check-in process and reduce wait times when you arrive at the vaccination site.
WHAT TO BRING TO YOUR VACCINATION APPOINTMENT
You will need to show a valid photo ID (university ID card, state-issued ID, or passport) and your insurance information if you have medical insurance. You will not be billed for the vaccination.
When you check in for your appointment, you will have the opportunity to register for your second shot at the on-campus site on the following days:
- Tuesday, May 18, from 9 a.m. to 4 p.m. in the Walb International Ballroom
- Wednesday, May 19, from 10 a.m. to 6 p.m. in the Walb International Ballroom
If you are receiving your second dose at the campus site, you must bring your CDC card showing proof of having received the first dose of the Moderna vaccine. You will receive this card when you receive your first dose. If you cannot make it to campus on one of these days for your second dose, you can schedule an appointment for a second dose at an alternate location as long as that location is administering the Moderna vaccine and it falls within the appropriate time frame—28 days after the first dose is recommended, but the second dose can be received up to 42 days after the first, according to the CDC.
OTHER IMPORTANT INFORMATION
- The Moderna COVID-19 vaccine is safe, highly effective, and free (if you have insurance, it will be charged a small administration fee, but no costs will be passed to you).
- Please be sure to wear your face mask to your vaccine appointment.
- If you have already made an appointment at another location for a COVID-19 vaccine, you may still register for a vaccination on campus, but please be sure to cancel your other appointment so that it can be given to someone else. If you registered through the state, you can cancel by calling 211.
- If you have already had COVID-19, you should still get a vaccine.
- It is OK to eat before your appointment.
- Please remember that Purdue Fort Wayne employees are not required to take sick or vacation leave for the time away from the office for the vaccine. Hourly employees should enter “other leave with pay” in Success Factors.
If you have questions about the COVID-19 vaccination site, please contact Jeff Malanson, the PFW COVID-19 point of contact, at [email protected].
