National Council of Licensure Examination (NCLEX) and State Board Application Information

Indiana State Board Application: The State Board requests applications be submitted on a timeline.  They will notify the school of when they want the online application submitted for our students.  Students will be notified of the timeline in class or via email.

Here are guidelines and instructions for submitting your online application to the Indiana State Board of Nursing:

  • To avoid security complications, it will be necessary to use either Chrome or Explorer.  Please DO NOT ATTEMPT TO APPLY USING A SMART PHONE OR TABLET.  
  • During the timeline specified for your school, go to Mylicense.IN.gov and click Register a Person to begin your application.  Complete the application and submit payment with a credit or debit card.  Allow 48 business hours for the application to be received and reviewed. If an applicant has a positive response to one of the questions on the application, the applicant will be contacted by a customer service representative with details of any additional required documentation (court documents, etc.), Documentation may now be uploaded to the website.
  • On the last page, the applicant is directed to go ahead and complete the Criminal Background Check.  Applicants will no longer receive a separate email from Indiana Professional Licensing Agency (IPLA) instructing them to complete the Criminal Background Check process.  Fingerprinting for the Criminal Background check is required.
  • With the new application, an ID photo is no longer necessary. 
  • Please make every effort to submit your applications during the requested timeline.
  • Upon your successful completion of the Nursing Program, the school will send your Certificate of Completion to the Indiana State Board of Nursing.  This is generally done after grades are submitted the week after finals.

National Council Licensure Examination (NCLEX): Also required is registration with the National Council of State Board of Nursing (NCSBN).

  • Go to NCSBN.org to fill out the online application
  • Use the IPFW B.S. Program Code: US48502000 (It is very important to use the correct program code)
  • Note: If searching for code by city, type in Ft Wayne
Virtual Assessment Technologies Institute (ATI) information:
  • Approximately one month before graduation, your name will be sent to ATI to get you started with Virtual ATI.
  • You will be contacted by ATI through your gmail email.  If you do not have a gmail account, you can set one up by going to google.com and clicking on gmail.  Then update your profile at atitesting.com with your new gmail email address.
  • ATI will send information for you to complete via your gmail address
  • ATI will contact you within two days of your returning the completed information.
  • 12-week access: students will have access to Virtual-ATI for 12 weeks, beginning once the student has completed their initial survey and is within 30 days of graduation. This will encourage students to review and sit for NCLEX sooner leading to higher pass rates and retention of knowledge. The student can purchase an additional 12 weeks, if necessary.
