M.S. Clinical Requirements

Health and clinical records (TB certification, physical, etc.) must be turned in to the Nursing Office, LA 345, by the deadlines listed below. Documentation must be good through the entire semester.  if you are not up to date with the requirements you may not begin clinicals. For any questions regarding clinical requirements please contact the nursing office, 260-481-6816.


Fall semester - August 15

Spring semester - December 7

Required Preceptor Documents

You will submit your preceptor forms to a dedicated Blackboard site.  The site contains the preceptor forms along with information about preceptors.  The preceptor list is updated periodically.

Deadlines for submitting preceptor forms are:
  • May 1 (fall semester)
  • October 1 (spring semester)
  • February 15 (summer I and II)

FNP and AGNP students should seek practicum experiences in primary care settings that match the focus of the course - Graduate NP Practica Guidelines

EDUC students should seek practicum experiences in practice settings that match the area of expertise and allow for provision of direct patient care.

Clinical requirements must be turned in to the Nursing Office in LA 345 by the policy deadlines.   Requirements may also be scanned and emailed to Jeni at or Sarah at with the exception of the drug screen results.  Drug screen results must be received in a sealed envelope -- either dropped off by student, or mailed directly from facility to the nursing school.
